Please fill out this form in order to inform both the Treasurer and the Administrator when a special collection is planned so that donations intended for that special collection can be correctly entered into our books and end up with the intended recipient.

We are asking everyone to use the form below every time there is a special collection, including for Generosity Sundays.

When you use the form, we can (1) set up Breeze to support the special collection, (2) book the special collection donations appropriately so the recipient gets everything that is intended for them, and (3) have an easily accessible record to refer to.
