Registration Form – This form is required to enroll your children in the RE program. Each family must complete this form once a year, preferably at the beginning of the RE year (September). If you need to complete the form, please print it out and return it to the fellowship by mail (address is on the form) or bring it to the Greeters table on Sunday.
Teacher Forms – Any adult who works with children in the RE program must complete these forms. This includes any adult who works as a teacher, facilitator, assistant, chaperone, etc. in the RE program. Please print the forms, complete them and return them to the fellowship by mail or place them in the DRE mailbox in the Office.
Field Trip Form – This form is required for any off-site field trip. Please complete the form and return it to the adult who is organizing the event.
